BIDMC, "The Advanced Heart Failure Clinic offers a multidisciplinary approach to ambulatory heart failure management and optimization of medication regimens. Patients receive one-on-one education from a nurse practitioner regarding heart failure, nutrition, exercise and lifestyle adjustments at initial and follow-up visits. Intensive management of symptoms and titration of medications are provided by phone between visits as needed. The team works closely with referring physicians, specialists and home health care agencies to optimize chronic incidence of hospitalization. Patients also have access to active congestive heart failure (CHF) clinical standard therapy. In addition, the clinic offers formal on-site assessment for cardiac transplantation through a collaborative program with New England Medical Center. The clinic offers access to the newest diagnostic and therapeutic modalities available for evaluation and treatment of heart failure patients, including cardiac MRI, novel drugs and devices, and an outpatient Nesiritide infusion clinic."
